A cheerful family sits together at a wooden table at home. A curly-haired woman wearing glasses and a striped top laughs joyfully, a young boy in a red and gray sweater smiles at a tablet screen, and a bearded man in a plaid shirt holds the tablet toward them. Colored pencils and open notebooks are spread across the table.

Birthmarks are unique skin features that can appear anywhere on the body and in a variety of forms, including vascular birthmarks such as port-wine stains, hemangiomas and venous malformations. While many birthmarks are harmless, their presence can be complicated by common skin conditions, adding complexity to diagnosis, treatment and patient comfort.

At The Vascular Birthmark Center in New York and Los Angeles, our team of specialists is dedicated to helping patients navigate these challenges with expert, individualized care.

Understanding Birthmarks: Types and Characteristics

Birthmarks can be broadly categorized into two groups: vascular (caused by abnormal blood vessels) and pigmented (caused by excess pigment). Common vascular birthmarks include:

  • Port-wine stains
  • Hemangiomas
  • Venous malformations

Pigmented birthmarks include café-au-lait spots and congenital melanocytic nevi. Each birthmark type may require different surveillance and management strategies, especially when other skin conditions are present.

How Common Skin Conditions Affect Birthmarks

Here are some ways that skin conditions can affect birthmark care:

Eczema (Atopic Dermatitis)

Eczema is a chronic, inflammatory skin condition marked by redness, itching and scaling. When eczema occurs on or near a birthmark, it can cause several complications:

  • Increased irritation and inflammation of the birthmark area
  • Difficulty distinguishing between eczema flares and changes in the birthmark, potentially delaying appropriate treatment
  • Potential risk of infection if scratching leads to broken skin, especially over vascular birthmarks

Psoriasis

Psoriasis is another chronic, inflammatory skin disorder characterized by thick, scaly plaques. When psoriasis overlaps with a birthmark:

  • Appearance of the birthmark may change, making monitoring for abnormal changes challenging
  • Treatment options may be limited, as some topical steroids or phototherapy could affect the underlying birthmark
  • Increased sensitivity or discomfort in areas where both conditions coexist

Allergic Skin Reactions (Contact Dermatitis, Urticaria, etc.)

Allergic reactions can cause redness, swelling and itching, sometimes leading to blisters or welts. When these reactions occur on or near a birthmark:

  • Inflammation may cause temporary changes in the color, shape or size of the birthmark
  • Some birthmarks, especially vascular types, may become more pronounced or irritated
  • Diagnosis can be complicated, necessitating expert evaluation to distinguish between allergic changes and birthmark progression

Other Common Skin Conditions

  • Fungal or bacterial infections can cause complications if they affect birthmarked skin, particularly if the skin barrier is already compromised.
  • Acne or seborrheic dermatitis may also present additional challenges in evaluating and treating birthmarks located on the face or scalp.

Implications for Treatment

When a birthmark coexists with another skin condition, the approach to treatment must be carefully individualized:

  1. Diagnosis: It may be difficult to differentiate between changes caused by the birthmark itself and those due to the skin condition. This can delay or complicate treatment.
  2. Treatment Limitations: Some therapies (like topical steroids or laser treatments) may not be appropriate for certain birthmarks or may require modification.
  3. Risk of Complications: The risk of infection, irritation or scarring may increase with overlapping conditions.
  4. Monitoring: Close observation is essential to ensure that treatment for the skin condition does not adversely affect the birthmark, or vice versa.

This underscores the importance of working with specialists who understand the nuances of both birthmarks and dermatological conditions.

5 Birthmarks and Skin Conditions FAQs

1. Can eczema or psoriasis make a birthmark worse?

Yes. The inflammation from eczema or psoriasis can increase irritation, change the appearance of the birthmark, and complicate monitoring or treatment.

2. Is it safe to use topical steroids on a birthmark affected by eczema or psoriasis?

It depends on the type and location of the birthmark. Some birthmarks may be sensitive to steroids or require alternative treatments. Always consult a specialist.

3. How do allergic reactions impact birthmarks?

Allergic reactions can cause temporary swelling or redness, making birthmarks look different and sometimes increasing discomfort or risk of infection.

4. Should I be concerned if my child’s birthmark changes appearance during a skin condition flare?

Yes—any significant change in a birthmark’s color, size or texture should be evaluated by a specialist, especially if accompanied by a new or worsening skin condition.

Dr. Levitin is very knowledgeable, compassionate and professional. Five years ago, I needed to find information about a birth mark that my newborn daughter started to grow a couple days after birth. I was afraid, I did not feel like I got any clear information about it through my insurance; also, they did not offer any course of treatment for it. I started to look for information, the birthmark was growing each day, information was scarce and I just got more scared. Finally, I found a video in Youtube from a grandmother who was very grateful after finding treatment for her grand daughter through The Birthmark Foundation. It was a huge blessing to me and my family. I found Dr. Levitin, I contacted him thinking he probably would not even answer. I was wrong, he kindly answered promptly, got in touch with me and soon he had a plan : ) I went to see him at the Birthmark Center in Los Angeles. He explained everything in a way that it was so clear to understand, no question I made was insignificant and his answers were each very informative. I really never have had an experience with a doctor who explains and speaks in such a way. I left his office knowing that my daughter was in the best hands and the best care. I first thank God for the Birthmark Foundation, a great organization with the highest quality of doctors. Also, a very special thanks to Dr. Levitin for all his help and kindness, for listening to my concerns and answering each and every question. And last, but no least, to the Birthmark Foundation Center in Los Angeles : ) I hope many children can find help/care in this place as I found it for my daughter. Thank you forever.

we are forever grateful for Dr. Levitin. My daughter Lena had surgery to remove her hemangioma that was located on her forehead at just 6 month old. she healed beautifully and we couldn't be happier with how minimal her scar is, even almost 4.5 years later. Dr. Levitin is caring, compassionate, and such a great doctor/surgeon overall. He had our daughter's best interest and I knew she was in good hands. I highly recommend him any chance I get!

Chris

My name is Chris and I was recently treated for an AVM on my forehead by Dr. Gregory Levitin of New York (New York Eye and Ear Infirmary). Thankfully, I live within a few hours of NYC. In brief, I would like to say that Dr. Levitin is very capable doctor with a lot of experience with AVMs. He responds quickly to e-mail, texts, and phone calls. He accepts insurance. He takes a "team approach" and will be your "QB" and involve other specialists as necessary. In my particular case they were Dr. Raj Shrivastava (Mt Sinai) and Dr. Raphael A. Ortiz (Lenox Hill) in NY. I recommend contacting Dr Levitin for a consultation if you suspect you have an AVM. All 3 Drs are just good people. When I spoke to them, they treated me like like an equal. I felt like they were advising me as a close friend or family member. Like many that suffer with AVMs I was at first mis-diagnosed. What was thought to be a just a simple cyst turned out to be an AVM. Surgery to remove the cyst made the AVM about 5x bigger. My treatment entailed a few MRIs and CAT scans to determine what was going on and the treatment plan. This was followed by consultation with the 3 Drs mentioned above and finally a 2-day procedure. On day 1 Dr Ortiz embolized the AVM. On day 2 Dr Levitin and Dr Shrivastava removed the AVM from my skull and put some metal mesh there to fill the "hole". It has been 6-months since the surgery and so far I have not had any recurrence. In honor of Vascular Birthmark Awareness Day and to thank these Drs for all they have done for me, I just want to give everyone a solid reference. Finding the right doctor for this type of stuff makes all the difference.
